General comments
Altijd rekening houden met het resultaat van eerder afgenomen kweken en recent antibiotica gebruik.
Bij ernstig verloop overweeg verwijderen/wisselen intravasale lijnen.
Indien de verwekker van sepsis bekend wordt, dient het gekozen antibiotica regime, voor zover mogelijk, gestroomlijnd te worden op basis van het antibiogram.
Denk bij kinderen onder de zes weken aan de mogelijkheid van neonatale herpes en overweeg start aciclovir na afname van HSV diagnostiek (PCR plasma, liquor, keelwat).
NB: Prematuren opgenomen sinds de geboorte met een kalenderleeftijd van >4 weken toch beschouwen als neonaat.
Let op verschil nosocomiaal en community acquired sepsis.
Dosering gentamicine: 4mg/kg elke 24 uur in 30 min laten inlopen. Na start dosering gentamicine onderhoud op eerstvolgende kalenderdag om 10.00 uur. NB voor gentamicine tijdschema is bewust van het kinderformularium afgeweken om praktische toepasbaarheid te verbeteren en foutgevoeligheid te verminderen. De hogere spiegels die hierbij kunnen voorkomen worden daarbij geaccepteerd.
